Abstract: SECTIONS 402(A)(6), 403(C), AND(J) OF THE SOCIAL SECURITY ACT PROVIDE FOR IMFORMATION REQUIRED TO ENSURE THAT APPLICANTS OR RECIPIENTS ARE NOT BEING DENIED AFDC, ADULT ASSISTANCE, OR MEDICAID COVERAGE FOR WHICH THEY ARE ELIGIBLE. THIS FORM PROVIDES A MORE RELIABLE, COST-EFFECTIVE MECHANISM FOR ASSESSING THE STATES' PERFORMANCE IN DENIAL OR TERMINATION OF COVERAGE. ENABLES SSA TO MAKE INCENTIVE PAYMENT TO THOSE STATES THA QUALIFY UNDER SECTION 403(J)
