Abstract: TITLE XVI OF THE SOCIAL SECURITY ACT PROVIDES FOR A WRITTEN SIGNED FROM THE CLAIMANT IN ORDER FOR CONDITIONAL SUPPLEMENTAL SECURITY INCOME (SSI) PAYMENTS TO BE MADE IN CASES WHERE RESOURCE LIMIT IS EXCEEDED. THIS FORM DOCUMENTS THE AGREEMENT TO DISPOSE OF EXCESS NONLIQUID RESOURCES WITHIN A SPECIFIED TIME PERIOD.
